Clinics

The Bristol Bay Area Health Corporation includes 21 village clinics that are accredited by Joint Commission and are staffed by Community Health aides. Our Community Health Aide Program (CHAP) plays a very important role in village health services. Our health aides can provide Preventative, Acute, Chronic, and Emergency care within their scope of practice using the Community Health Aide Manual (CHAM) and reporting to a referral Physician at the Kanakanak hospital. They work closely with our case management and assist many other departments that include, but not limited to: Optometry, Audiology, Diabetes, Medical Staff, Dental, and Infant learning to ensure our patients are getting the appropriate and essential care that they need.

 

Care Teams

There are three Care Teams at BBAHC: Bear Team, Salmon Team, and  Sea Otter Team. Each team consists of providers, health aides, RN Case Managers, and support staff who are dedicated to providing optimal care that is timely and cost-effective. The Care Teams strive to promote health and quality of life, bridge community services along the continuum of care, and provide support for nearly any type of need.

 

Case Management

Case Management works with the patient and their medical provider to create a personalized care plan through:

  • Assessments – Determining any barriers to care
  • Linkages and Referrals – Helping to connect patients with appropriate services
  • Care Planning – Developing a personalized health care plan
  • Patient Advocacy – Helping patients work through paperwork, talking to other organizations, and being an available support team
  • Monitoring and Outreach – Making sure patients are getting the most out of their visits
  • Collaboration and Consultation – Working with all Care Team providers to help ensure patients get the best health care possible.

CHAP villages: Aleknagik, Chignik Bay, Chignik Lagoon, Chignik Lake, Clarks Point, Egegik, Ekwok, Goodnews Bay, King Salmon, Koliganek, Levelock, Manokotak, Naknek, New Stuyahok, Perryville, Pilot Point, Platinum, Port Heiden, South Naknek, Togiak, Twin Hills.

 

We also provide TDY EMT coverage in Ekuk, Ugashik, and Igushik.

Bristol Bay Area Health Corporation manages Kanakanak Hospital, a critical access facility, which is Joint Commission accredited, receives Health and Human Services funding, and is covered through the Federal Tort Claim Act with respect to certain health or health-related claims, including liability for medical malpractice claims, for its officers, board members, employees, and certain health profession contractors and health profession volunteers. Bristol Bay Area Health Corporation is recognized by the US Government and State of Alaska as a tribal organization under PL 93-638.
