Case Management Village Coverage Update!

The Bristol Bay Area Health Corporation's Case Management Department has three patient-centered care teams ready to assist you!  The Bear, Salmon and Sea Otter teams each have a dedicated registered nurse and support staff to provide service to selected villages and providers.  Patients now have a team of professionals for continuity of care and a consistent point of contact.  Our teams will get to know you, understand your needs, and track your upcoming appointments.  Look for a postcard in the mail or click on the flyer below for information on your new care team.  Be sure to make note of your Case Management Team RN and support staff phone numbers and reach out when we can assist you with your health care needs.  We look forward to serving you with competence, compassion and sensitivity.
