The BBAHC Medical Social Worker assists patients and families with safe hospital discharges, finding long term placement homes, and assuring they have appropriate equipment in their homes for safe environment. The social worker advocates for patient and family needs, including legal documents, advance care planning and safety.

- Advance Directives
- Physician Orders for Life Sustaining Treatment (POLST) forms
- Power of Attorney
- Report of Harm to Seniors
